Participation in K Dance Studio classes require completion of this form.

I understand risks in dancing, and by signing this Waiver, I voluntarily assume all such risks and release the K Dance Studios and their related parties from any liability for injuries or losses that may occur.*

I represent that I am physically fit and have no medical conditions that prevents my safe participation in dance activities. It is my responsibility to consult with a doctor before participating where required.*

I agree to comply with all rules, instructions, and safety guidelines provided by K Dance Studios regarding dance activities. I understand that failure to adhere may result in my removal from classes without refund.*

I grant K Dance Studios permission to use any photographs, videos, or audio recordings of me taken during classes for promotional and marketing purposes.
